Our Intake Coordinator will be in contact with you within 2 business days after reviewing your information and if you qualify for the program, you will be sent our full application to complete. Please check your email frequently. Please also check your span or junk mail folders.
We will need the following prior to scheduling an interview:
- Application completed in its entirety
- Valid ID/Driver's license for all adults in the household
- Social security cards and birth certificates for all household members
- Paystubs from the last 30 days, reflecting a minimum of 30 hours/week, and proof of all other income
- Proof of homelessness
